Our Approach to Interventional Pain Therapy

Interventional pain therapy is built on a minimally invasive philosophy: identify the precise structure generating the pain and target only that source. The goal is to reduce reliance on broad, systemic medication, accelerate recovery and help patients return to daily life sooner. Every procedure is delivered within a personalized plan grounded in careful examination and imaging findings.

Image guidance is central to this approach. Delivering medication to the exact target under fluoroscopy or ultrasound improves both effectiveness and safety, protecting neighbouring tissues and keeping unwanted effects to a minimum. Precision is what distinguishes a targeted intervention from a general treatment.

These procedures are not a standalone cure but one part of a comprehensive plan. They are considered alongside physiotherapy, exercise, lifestyle adjustments and, when needed, medication. The degree and duration of relief vary from person to person, so realistic expectations and regular follow-up are essential to lasting results.

Chronic Low Back and Neck Pain: A Diagnosis-Led Approach

Chronic low back and neck pain may arise from the disc, facet joints, a nerve root or muscle. Effective treatment is only possible with an accurate diagnosis. The process therefore begins with a detailed assessment of the pain's character, radiation and triggers, supported by MRI, CT and, when appropriate, diagnostic blocks.

The evaluation interprets imaging in the context of the patient's history and physical findings rather than in isolation. Because the same imaging finding can cause very different symptoms in different people, treatment decisions are made with a holistic perspective.

Once the diagnosis is clear, care begins with the least invasive options. The aim is to avoid unnecessary surgery, bring pain under control and restore the patient's capacity to move. Interventional techniques are an important tool within this stepwise strategy.

Epidural and Transforaminal Injections

Epidural and transforaminal steroid injections are used when a nerve root is compressed or inflamed, particularly for pain that radiates into the leg or arm. Under fluoroscopic guidance, medication is delivered precisely around the affected nerve to reduce inflammation. This is a well-established option for referred pain caused by lumbar or cervical disc herniation.

The transforaminal technique allows more selective delivery to the affected nerve root. Image guidance ensures the correct level and side are targeted, providing diagnostic information while supporting therapeutic effect. The procedure is usually brief, and patients can typically return home the same day.

The relief provided varies according to the individual and the underlying problem; it may be long-lasting for some and temporary for others. Importantly, an injection creates a valuable window in which a physiotherapy and exercise programme can be activated for more durable improvement.

Facet Joint Blocks and Radiofrequency

The facet joints of the spine are a common source of back or neck pain that worsens with movement and becomes prominent in certain positions. A diagnostic facet joint block helps confirm whether these joints are genuinely the source of the pain and guides the treatment plan accordingly.

When a block confirms the source, radiofrequency denervation (thermocoagulation) may be considered. In this technique, the small nerve branches carrying pain signals from the joint are targeted with radiofrequency energy, selectively reducing pain transmission. The procedure is performed under image guidance in a controlled, minimally invasive manner.

The effect of radiofrequency denervation can often last for many months, and the procedure may be repeated over time as the nerve regenerates. In carefully selected patients, this approach aims to reduce the need for medication and support a return to everyday activity.

Ganglion Impar and Other Targeted Procedures

Some pain syndromes are addressed more effectively by procedures that target specific nerve structures. Persistent pain at the tailbone, known as coccydynia, is a good example. A ganglion impar block and radiofrequency thermocoagulation aim to provide relief by targeting the nerve network that carries pain signals from this region.

Such interventions are considered in patients who struggle to sit, have long-standing symptoms and have not responded adequately to other treatments. The procedure is performed under image guidance, reaching the relevant nerve structure with precision, and is usually carried out on a day-case basis.

Beyond ganglion impar procedures, sympathetic blocks and other targeted nerve techniques for different regions may also be added to the plan. Each procedure is personalized to the patient's individual pain map and overall health.

Regenerative Pain Approaches and Realistic Expectations

Regenerative approaches are an emerging field in pain management. Spinal stem-cell and exosome injections are being studied as methods that aim to modulate inflammation within tissue and support repair processes. These applications are considered as a complementary option to established interventional techniques, not a replacement for them.

It is important to be clear that these treatments are not suitable for everyone and do not promise miraculous results. Effectiveness and duration vary between individuals, and the scientific evidence continues to evolve. For this reason, regenerative options are raised only after a detailed evaluation and in patients for whom they are deemed appropriate.

The decision is shaped by the patient's diagnosis, response to previous treatments and a frank discussion of realistic expectations.
